The Birches At Home was developed to meet the growing desire of individuals to receive assistance with the everyday tasks and still live in their own homes.Whether aiding you with daily living tasks, travelling or running errands, 'The Birches At Home' home care services can help. The Birches At Home is privately owned. It is managed by Heritage Senior Living, who manages senior living facilities in the mid-Atlantic region.
The Birches At Home provide individualized care solutions for your needs in your own home. You will receive a comprehensive health and needs assessment and participate in creating your own care plan. The care plan is re-evaluated on an ongoing basis and readjusted as your needs change.
The Birches At Home Care Plans include:
Respite care (planned temporary relief for the family caregiver)
Emergency care (for individuals recovering from illness or surgery)
Transportation and care after discharge from hospital/rehab
